When to use
- Clinical staging of CLL at diagnosis using physical exam and a routine blood count only (no imaging or marrow required).
Formula
Areas counted: cervical, axillary, and inguinal nodes (each counted once regardless of laterality), spleen, liver. Stage A: <3 areas, no anemia (Hb<10) or thrombocytopenia (platelets<100x10^9/L). Stage B: ≥3 areas, no anemia/thrombocytopenia. Stage C: anemia and/or thrombocytopenia regardless of areas involved.
Pearls and pitfalls
- Anemia and thrombocytopenia are the dominant prognostic factors and override nodal area count.
- The Rai staging system is the historical US counterpart; both remain in use alongside molecular markers (IGHV, FISH, TP53).
