When to use

  • Stratifying patients with C. difficile infection at diagnosis to predict likelihood of clinical cure and help select therapy.

Formula

Sum of 5 items, each 0-2 points except antibiotics (0 or 2): Age (<60=0, 60-79=1, >=80=2), concurrent systemic Antibiotics (no=0, yes=2), Leukocyte count (<16=0, 16-25=1, >25 x10^9/L=2), Albumin (>35=0, 26-35=1, <=25 g/L=2), Serum creatinine (<=120=0, 121-179=1, >=180 umol/L=2). Range 0-10.

Pearls and pitfalls

  • Derived and validated on two large fidaxomicin-vs-vancomycin trial databases; very ill patients (e.g. fulminant colitis, ICU) were under-represented.
  • Not a severity score in itself, but a predictor of response to standard antibiotic therapy.
