When to use
- Guiding statin-intensity decisions in adults with or without known ASCVD, per the 2018 AHA/ACC/multisociety cholesterol guideline risk thresholds.
Formula
If clinical ASCVD is present, secondary-prevention statin intensity is assigned directly. Otherwise the Pooled Cohort Equations (see ASCVD 2013 calculator) estimate 10-year risk, which is mapped to statin intensity using the <5% / 5-<7.5% / 7.5-<20% / >=20% thresholds.
Pearls and pitfalls
- Very-high-risk secondary-prevention patients may warrant LDL-C targets <55-70 mg/dL with add-on non-statin therapy.
