Appendicitis inflammatory response (AIR) score
Risk-stratifies suspected appendicitis using clinical and inflammatory markers.
Appendicitis inflammatory response (AIR) score
Vomiting
Right iliac fossa pain
Rebound tenderness or muscular defence
Body temperature ≥38.5 °C
Polymorphonuclear leukocytes (neutrophils)
White blood cell count
C-reactive protein
Result0 points
Score 0–4: low probability — outpatient follow-up may be appropriate.
When to use
- Stratifying suspected appendicitis, including in children, to separate low-risk from high-probability patients.
Formula
Vomiting 1, RIF pain 1, rebound/defence (light 1, medium 2, strong 3), temperature ≥38.5 °C 1, neutrophils (70–84% =1, ≥85% =2), WBC (10–14.9 =1, ≥15 =2), CRP (10–49 =1, ≥50 =2). Range 0–12. 0–4 low, 5–8 indeterminate, 9–12 high.
