When to use

  • Community or primary-care self-screening for undiagnosed type 2 diabetes before ordering laboratory testing.

Formula

Sum of points: age (<40=0,40-49=1,50-59=2,>=60=3) + sex (F=0,M=1) + gestational diabetes history (1) + family history (1) + hypertension (1) + physical inactivity (1) + BMI category (<25=0,25-29.9=1,30-39.9=2,>=40=3). Score >=5 flags increased risk.

Pearls and pitfalls

  • Derived from the NHANES-validated Bang score; the ADA's public-facing 'Diabetes Risk Test' uses this same item set.
  • A screening tool only - does not replace fasting glucose or HbA1c for diagnosis.
